Pre-authorized Payment Authorization Form

To enroll in one of two (2) pre-authorized payment plans options please fill out the application form below and submit with a picture of your VOID cheque. Treasury staff will review your application and confirm via email when you have been successfully registered on a payment plan. The Township of South Frontenac requires a completed application at least fifteen (15) days before the first automatic withdrawal.

A separate application form must be completed for each individual property that you would like enrolled.

You will continue to receive tax bills showing you the amounts billed to your property. Your bill will state that you are on a pre-authorized payment plan.

  1. I/we authorize the Township of South Frontenac to debit my/our account as indicated on the attached VOID cheque under the terms and conditions agreed to by me/us with the payee until such time as written notice to the contrary is given.

  2. I/we acknowledge that delivery of my/our authorization to the Township constitutes delivery by me/us to the branch of the financial institution at which I/we maintain an account and that such financial institution is not required to verify that the payment(s) are drawn in accordance with this authorization. Termination of this authorization does/may not terminate the contract for goods or services exchanged.

  3. I/we will notify the Township in writing of any changes to the account information or termination of this authorization (ie. sale of property) 15 days prior to the next due date of the pre-authorized debit, which I acknowledge as being the due date indicated on my payment plan application.

  4. A charge will be made by the Corporation of the Township of South Frontenac (and added to the tax account) in the event any cheque or other order is not paid by the Financial Institution or any electronic transfer is not completed due to insufficient funds in the account or any other reason.

  5. The Corporation of the Township of South Frontenac may, at its sole option, cancel or suspend the right to pay the tax account pursuant to the authorization without notice.

  6. Supplementary/Omitted tax bills or utility bills added to the tax account cannot be paid through this program and must be paid by a separate payment.

  7. Note that depending on the plan selected and the time of year enrolled, you may be required to make a payment prior to enrollment in the plan. Please ensure you check your email until a confirmation email has been received. 

  8. Accounts where a mortgage holder remits taxes on behalf of the owner are not eligible for pre-authorized payment plans.
